Important Reminders:

HMO Patients: All HMO patients require prior authorization for appointments. Check with your health plan or medical group.

Medicare Patients: We can treat patients enrolled in the regular Medicare program (fee-for-service Medicare), with or without physician referral. However, if you are enrolled in a Medicare HMO, you must obtain prior authorization to see one of our doctors. Check with your health plan or medical group. 

PPO and Point of Service Patients: Check with your plan to determine if the FPP faculty specialist you wish to see is a network provider. Remember, you will be responsible for all costs not covered your insurance, including your deductible and co-payment, and any differential charges pertaining to use of non-network providers.

Self-Pay Patients: If you plan to pay for your visit yourself, notify the receptionist when you schedule your appointment.
